| Instructions for Opening Your New Share (Savings) Account
Please Note: As part of the USA Patriot Act, Federal law requires
us to obtain, verify, and retain information regarding the identity
of anyone that has an account with The Byzantine Federal Credit
Union.

to download the Byzantine Federal Credit Union's New Membership
Application. Please print this form (front & back) then complete and
sign where indicated.
In order to verify the identity of our members, including all
joint owners, a non-expired, government-issued photo identification
card (ie - driver's license, military ID) MUST be provided before we
can open this account. (Please be sure that ID photocopies can be
seen clearly). If you do not have a government-issued photo ID, two
alternate pieces of ID are required. This can be a Social Security
card, birth certificate, state issued ID or pay stubs. If you are
not a U.S. citizen, please call the Main Office for further
instructions at 216.351.4353.
Enclose a check or money order for at least $25.00 to establish
your Regular Share (Savings) Account with The Byzantine Federal
Credit Union. Please make check or money order payable to: The
Byzantine Federal Credit Union.
Please send the completed Application for Membership, Photo IDs
(or two alternate pieces of ID) for all primary and joint owners,
and a check or money order for at least $25.00. Send to: Byzantine
Federal Credit Union, 4682 State Rd., Cleveland, OH, 44109 or
bring all of these items in person to our location during normal
business hours.
